  4. Eligibility probably rating (Low/Possible/Strong).

  5. A personalised written action plan including: what evidence you need to gather, whether and when to request a checklist, what to expect and how to prepare for the process, key risks and how to avoid common mistakes.

This service is ideal for:

  • Families new to CHC and looking for direction

  • Those told they are “not eligible” but feel differently

  • Loved ones recently discharged into a care setting

  • Anyone wanting to avoid delays or mistakes in the process.

What you will need to provide

To complete this assessment quickly and accurately, we will ask for:

Documentation (where available)

  • A recent care home or hospital care plan

  • Latest nursing/care home notes

  • GP summary or discharge letter

  • Current medication list

  • Any NHS CHC documentation

  • Any Local Authority records

  • Any other specialist assessments (including but not limited to OT, physio, SALT, Mental Health)

Family Perspective

  • Your description of daily needs and challenges

  • Any incidents/events you feel are important

  • Changes in health or behaviour over time

ICB or Local Authority details (if a request or CHC decision has been made previously)

Don’t worry if you don’t have everything, guidance and support will be provided every step of the way, including help on how to request missing information and what is most important.

Outcome you can expect

Within 5 working days of receiving your documentation you will receive:

  • A written assessment report

  • Practical next steps clearly mapped out

  • Optional upgrade recommendations if CHC eligibility is likely.

This ensures the case is built properly from the start, increasing your chance of a positive outcome.
