What you will need to provide
To complete this assessment quickly and accurately, we will ask for:
Documentation (where available)
A recent care home or hospital care plan
Latest nursing/care home notes
GP summary or discharge letter
Current medication list
Any NHS CHC documentation
Any Local Authority records
Any other specialist assessments (including but not limited to OT, physio, SALT, Mental Health)
Family Perspective
Your description of daily needs and challenges
Any incidents/events you feel are important
Changes in health or behaviour over time
ICB or Local Authority details (if a request or CHC decision has been made previously)
Don’t worry if you don’t have everything, guidance and support will be provided every step of the way, including help on how to request missing information and what is most important.
Outcome you can expect
Within 5 working days of receiving your documentation you will receive:
A written assessment report
Practical next steps clearly mapped out
Optional upgrade recommendations if CHC eligibility is likely.
